I've seen this also with my Norco 12 bay sata boxes, I've always believed the two failure conditions are related to how the drive has failed.
1. Drive Gone - Drive electronics have failed or need a power cycle to reset some sort of firmware issue.
2. Drive having major IO errors but not gone - This is a failure of the physical media but the interface in the drive still works.

I could be wrong, I've had my systems for about 5 years now and I've seen these two conditions a good number of times.
Condition One will mean dead drive or power cycle might fix.
Condition two will mean always replace drive.

Cheers
